On November 27, 2026, the NCCC will submit evidence to assist the Tribunal's decision on long-term reform (outside Ontario): we need to hear from you to inform that evidence.

The NCCC needs to hear directly from Nations and Agencies about what is happening on the ground. Please fill out the forms below as soon as possible to help us submit evidence to the CHRT. This evidence will help the Tribunal as it makes its decision to issue Long Term Reform orders.

The questionnaires below focus on the areas that we (and the CHRT) are missing the most information. They take about 20 minutes to fill out.

If you are authorized to speak about child & family services on behalf of your First Nation, please complete the Survey for First Nations

If you are authorized to speak on behalf of a First Nations Child & Family Services agency, please complete the Survey for FNCFS Agencies

Thank you all for any help you may be able to provide - as we near the end of this 20-year journey we can finish strong if we finish together.

If your Nation or Agency is willing to take a look at a more detailed questionnaire please email nccc@nationalchildrenschiefscommission.org and we will get you a copy. You can also email the NCCC if you have additional information about impacts at the level of the child that does not fit in the forms.

All information collected will only be used to bring evidence to the CHRT in anonymized (statistical) format unless an organization gives written permission for their name to be used. It will be stored securely on the Our Children Our Way servers with extremely limited access to identifying information. Only anonymized data will be used by the broader technical team to undertake analysis.

The NCCC brought to you by Resolutions 60 and 61 of the Special Chiefs Assembly, October 2024
